In a cocktail shaker or mason jar combine the cold brew coffee and the chocolate syrup. Close the lid tightly and shake to combine.
1/2 cup cold brew, 2-3 tbsp chocolate syrup
Pour in a glass full of ice, top it off with the milk, some whipped cream if you'd like and enjoy!

Notes
Shake, do not stir. Nothing in this drink is warm, so chocolate syrup will not dissolve by stirring the way it does in a hot mocha. Ten seconds in a sealed jar with the cold brew, before any ice goes in, is what keeps the chocolate suspended instead of sitting on the bottom. Use ready to drink cold brew, not concentrate. If you only have concentrate, dilute it to drinking strength per the bottle first, then measure your half cup. Weak coffee is the single most common reason this drink comes out watery.
